Case Management Action Group - Disability Information Kit

The Case Management Action Group (CMAG) is a collective of, primarily, disability case management agencies operating in the North and West region of Metropolitan Melbourne. Currently, CMAG has members from 45 different agencies, both government and community service organisations including disability case management, community health, carers groups and service user support and advocacy groups.

Who is the Information for?
The information contained in the sheets is intended for people with a disability and/or their families and carers who are new to the disability service system or are after a little extra information. This is not to replace other services, such as case management, but to give people the opportunity to access services and resources independently if so desired.

The Design of the Information Sheets

The information in the kit is organised around key transition stages of a person’s
life. The information falls into the following groups:
1.1 – 1.4 0 years to school aged
2.1 – 2.5 Young Adults
3.1 – 3.6 Older Adults
4.1 – 4.8 Information relevant across all ages

How to use the Kit

The information compiled in these sheets has been broken down to cover the information most commonly required at key life stages e.g. from choosing a school to information regarding accommodation options for an older adult.

Please note that all documents are in pdf format and require acrobat reader.
